Teaching and Substitute Candidates must submit the following:

• Standard State Application and Resume, completed in full

• All Transcripts

• Act 34 Clearance

• Act 151 Clearance

• Act 114 The Federal Criminal History Record

• Act 168 Clearance - Candidates that have previous contact with children need to start the form and then give it LH and we will send it directly to their previous employer.

• PDE 6004

• Teaching Certification(s) or Letter from Dean of Certifying Institution

• Proof of passing any appropriate National Teacher Exams

• QPA Form

• Physical Exam

• TB Tine Test or Chest X-Ray Results

• (3) Letters of Recommendation

 

Custodial, Maintenance, Cafeteria, Secretary, Security, Teacher Aide, and Health Room Aide (Support Personnel) Candidates must submit the following:

• Employment Application For Support Personnel

  • Act 34 Clearance

  • Act 151 Clearance
